  Pinnacle Home Care is committed to making a difference in the lives of our patients and their families. We are seeking a dedicated and compassionate Physical Therapist to join our team.
  The Physical Therapist will provide comprehensive assessments, develop treatment plans, and implement physical therapy programs to improve patient outcomes. The ideal candidate will have a strong commitment to quality care and a passion for working with patients in their homes.
  Responsibilities:
  
  • 
    
  • Conduct initial and ongoing comprehensive assessments of patients' needs
    
  • Develop and implement treatment plans in consultation with physicians
    
  • Teach, supervise, and counsel patients and their families on physical therapy programs
    
  • Communicate with physicians and other healthcare professionals to ensure seamless care
    
  • Write clinical and progress notes on patient records
    
    Qualifications:
    
    • 
      
    • Graduate of an accredited Physical Therapy program
      
    • Licensed to practice as a PT in the State of Florida
      
    • Current BLS Certification
      
    • At least one year of experience as a Physical Therapist
      
    • Home healthcare experience preferred but not essential
      
    • Electronic medical record experience preferred but not essential
