**Job Description**

As a Physical Therapist with Pinnacle Home Care, you will provide comprehensive assessments of patients' needs, establish treatment plans, and teach and supervise family members and caregivers. You will also communicate with physicians and other healthcare professionals to ensure seamless care.

**Responsibilities**

--- Provide initial and ongoing assessments of patients' needs
--- Establish and revise treatment plans as necessary
--- Teach and supervise family members and caregivers
--- Communicate with physicians and other healthcare professionals
--- Supervise Physical Therapist Assistants
--- Write clinical/progress notes and maintain accurate records

**Qualifications**

--- Graduate of an accredited Physical Therapy program
--- Licensed to practice as a PT in the State of Florida
--- Current BLS Certification
--- At least one year of experience as a Physical Therapist
--- Home healthcare experience preferred but not essential
--- Electronic medical record experience preferred but not essential
--- Must display good emotional health and be able to physically tolerate much standing, stooping, bending, and heavy lifting
