Job SummaryWe are seeking a skilled Physical Therapist to join our team at Avante at Mount Dora, a Skilled Nursing Facility. As a Physical Therapist, you will provide and direct Physical Therapy services to residents, including assessment, treatment, program planning and implementation, related documentation, and communication.Responsibilities

• Provides Physical Therapy services to residents and initiates referrals when appropriate and in accordance with department procedures.
• Conducts screenings of residents at regular intervals to determine the need for intervention/treatment.
• Evaluates residents to obtain data necessary for treatment planning and implementation.
• Conducts specialized evaluations as indicated.
• Adheres to established confidentiality standards.
• Implements and conducts treatment as outlined in treatment plan.
• Follows management direction.
• Performs other duties as assigned.Requirements
  • Bachelor of Science in Physical Therapy from an accredited program.
  • Successful completion of National Certification Examination for Registered Physical Therapist.
  • Current state licensure or license eligible.
