Job SummaryMederi Caretenders Home Health is seeking a skilled Licensed Practical Nurse (LPN) to join our team in a Full-Time or PRN position. As a LPN, you will provide high-quality patient care and perform technical skilled care in the patient's home.Responsibilities

• Provide quality patient care and perform technical skilled care in the patient's home, following all applicable infection control procedures.
• Continually observe and report the patient's nursing needs to supervising RN.
• Communicate timely and effectively with physicians, patients, family members, and appropriate agency staff regarding any patient care issues or needs.
• Document patient visits thoroughly and completely per policy and payer requirements.
• Actively participate in the performance improvement plan and work to reduce patient hospitalizations and improve outcomes.Requirements
  • Current LPN licensure in state of practice.
  • Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
