Job Description:

A company is looking for an Arizona Long Term Care (ALTCS) Case Manager.

Key Responsibilities

Assess and document the functional, physical, and behavioral health status of assigned members
Develop and implement service plans based on member strengths and needs
Act as a facilitator and advocate for members in dealing with providers and community programs

Required Qualifications

Bachelor's degree in social work
Two years of experience serving elderly individuals or those with physical disabilities or Serious Mental Illness (SMI)
Residency in Pima County, Arizona
Preferred bilingual skills in some assignments
Additional related education and/or experience preferred
